If you wish to register with Howick Health & Medical Centre, you will need to fill out an enrolment form, which can be downloaded below.
During the current COVID 19 outbreak it would be preferable that you fill the forms to your right at home and email them to reception at howickreception@outlook.com. We are trying to minimise the amount of foot traffic in the reception waiting room given the current outbreak.
Before you turn up please ensure the following are filled in:
an enrolment form (on the right) and
a medical information form (on the right).
also please provide us with a copy of your passport page with your photo or a birth certificate
Completion of these forms will help inform your doctor about your medical history and allow us to communicate with you. Alternatively, feel free to print these forms out and complete them prior to coming in. The email and mobile communication form is to be read at the time of enrolment.
If you are not enrolled with our practice, we will not receive funding from the Ministry of Health. In which case, you will notice your consultation fee will be more expensive.
Information about your intent to be enrolled as a regular patient will be sent to ProCare and the Ministry of Health to secure subsidies.
This practice transfers information electronically and in printed form. Procedures are in place to ensure information is sent securely to adhere to strict privacy legislation. This notice is to inform you of the information transferred from this practice.
Casual Patients If you intend to attend our medical centre as a casual patient please fill in the casual enrolment form (click here) and the medical information form (click here) Then email these 2 forms with copy of your passport page showing a photo of yourself and your details. to howickreception@outlook.com. Please allow at least 24 hours for the information to be entered into our system before calling to make an appointment.
Housekeeping Tips
Kindly turn off your cell phone during consultation time.
Unpaid accounts will incur an $8.00 administration fee if not paid at the time of consultation.
If extra time is required, please indicate this to reception. Please note: Consultation time is for 15 minutes only. Any additional time spent will be charged for.
Prescriptions, reports, blood pressure checks, materials used and time spent with the nurse will incur a charge. More information is displayed on our reception desk and on the fees page of this website.
