Kindly turn off your cell phone during consultation time.
Unpaid accounts will incur an administration fee if not paid at the time of consultation.
If extra time is required, please indicate this to reception. Please note: Consultation time is for 15 minutes only. Any additional time spent will be charged for.
Prescriptions, reports, blood pressure checks, materials used and time spent with the nurse will incur a charge. More information is displayed on our reception desk and on the fees page of this website.
Test results: Normal results are not usually conveyed to patients however you can access your results via the portal.
Please complete the Enrolment Form in full and follow instructions below.
Please complete ALL fields otherwise the enrolment will be incomplete and may delay funding for your visit.
Online:
Please complete ALL fields of the online enrolment form.
Choose a provider or providers you wish to enrol under. You can choose more than one.
We need to know your contact details AND the consented methods of contact.
Please advise who the account holder is for the patient.
You must advise of your current practice details so a transfer of notes can occur for continuity of care.
You must provide ID.
Please email to reception@howickhealth.co.nz the 'medical information form' as well.
In-person:
First, click on the Medical Information Form link, print out the form, and then fill out the form.
When you come into the clinic, please bring a copy of your passport page with your photo, or a birth certificate, along with the completed Medical Information Form.
We require this documentation for the registration/enrolment process.
If you are not enrolled with our practice, we will not receive funding from the Ministry of Health.
In this case, you will notice your consultation fee will be more expensive.
Casual Patients
We do not accept casual patients. You must be an enrolled patient to be seen.
