Online: 1. Please complete ALL fields of the online enrolment form. 2. Choose a provider or providers you wish to enrol under. You can choose more than one. 3. We need to know your contact details AND the consented methods of contact. 4. Please advise who the account holder is for the patient. 5. You must advise of your current practice details so a transfer of notes can occur for continuity of care. 6. You must provide ID please. 7. Please email to firstname.lastname@example.org the 'medical information form' as well please.
In person: Before you turn up please bring a copy of your passport page with your photo or a birth certificate along with a completed medical information form:
then come in to complete the registration/enrolment process by signing the enrolment form and bringing in a completed medical information form.

If you are not enrolled with our practice, we will not receive funding from the Ministry of Health. In which case, you will notice your consultation fee will be more expensive.
Information about your intent to be enrolled as a regular patient will be sent to ProCare and the Ministry of Health to secure subsidies.
This practice transfers information electronically and in printed form. Procedures are in place to ensure information is sent securely to adhere to strict privacy legislation. This notice is to inform you of the information transferred from this practice.
Casual Patients If you intend to attend our medical centre as a casual patient please fill in the casual enrolment form (click here) and the medical information form (click here) Then email these 2 forms with copy of your passport page showing a photo of yourself and your details to email@example.com. Please allow at least 24 hours for the information to be entered into our system before calling to make an appointment.

Unpaid accounts will incur an $10.00 administration fee if not paid at the time of consultation.
If extra time is required, please indicate this to reception. Please note: Consultation time is for 15 minutes only. Any additional time spent will be charged for.
Prescriptions, reports, blood pressure checks, materials used and time spent with the nurse will incur a charge. More information is displayed on our reception desk and on the fees page of this website.
