Then either call or book an appointment with your doctor online. Please make a double booking for this. You will not be charged.
If you cannot send these forms to us that is ok as your doctor will ask these questions over the phone/ video consult.
Flu vaccines 2020: Flu vaccines have arrived and are available currently for those who are funded to receive the flu vaccine - click here to see if you are eligible. Those who are not eligible will have to wait until April 13. You can book online (to see the nurse) or phone.
Novel coronovirus (Covid-19): If you have been overseas within the last fortnight or you have cold symptoms (ie 1 or more of the following- fever, runny nose, cough, headache, muscle aches, fatigue) PLEASE let reception know at the time of booking your appointment. From January Dr. Ivy Chen unfortunately will no longer be working however Dr Ruby Yeh who many of you will have met will be joining us permanently from March 2 2020. If you were under the care of Dr. Chen you can register under one of the other doctors. Both Amanda and Hayley have recently increased their availability.

Dr. Hayley Wilson is now also available on Monday mornings 9-12pm from July 29. This is in addition to her Tuesday am session and Thursday whole day. Dr Amanda Sew Hoy will work an additional session and will be available on Mondays, Wednesdays and Fridays.

Patient Portal: If you are a registered patient you are now able to access your results on the patient portal by clicking on the 'Patient Portal' link to the right and selecting the 'Health Record' tab. You will need to complete a consent form first.
